Membership in the Florida Society of Neonatologists is restricted to Board-certified and board-eligible neonatologists in Florida. Associate membership is available for physicians who are currently in fellowship training programs in neonatal-perinatal medicine. If you are interested in applying for membership in FSN, please complete the following steps in the application process:
- Fill out the Application form. Application forms may be submitted electronically, or they may be printed and sent to us by fax (904)549-3053 or mail (Florida Society of Neonatologists, c/o Dr Mark Hudak, Dept of Pediatrics, 953-1 West 8th Street, Jacksonville, FL 32209).
- Send in either:
a) copy of certification certificate by sub-bosrd of Neonatal-Perinatal medicine.
b) Proof of Board eligibility; or
c) Letter from Program Director stating that you are actively nvolved in a neonatology fellowship training program.
- Also please submit a copy of your current Curriculum Vitae, and a check for $50 made out to FSN for your first year dues.
