Assessment of Relative Benefit of HFOV in Preterm Infants of Differing Severity and Maturity: A Logistic Regression Meta-Analysis. Bachman T, Ashurst JT, and Gerstmann DR. Provo, Utah.

This meta-analysis looked at data from 468 premature babies enrolled in 6 clinical trials comparing the Sensormedics 3100A high frequency ventilator to CV for treatment of RDS. Findings included a decreased risk of death or CLD with HFOV (p=.01) and a comparable risk of severe IVH (p=.854). An important new finding was a decreased relative risk of severe IVH with HFOV compared with CV as infant’s severity decreased (p=.018).

Comment. This should put to rest concerns about possible increased risk of severe IVH with HFOV that have lingered since the original HiFi Study. It seems clear now that at least with this device used with an optimal mean lung volume strategy there is no increased risk of severe IVH.
